One consultation visit is free of charge. A 24 hour notice is required to reschedule a consultation appointment. If the appointment is a “no-show”, a $50 cash prepaid deposit is required to schedule another consultation. The $50 deposit will be applied toward any procedure of choice.
Please arrange for child care and do not bring children to appointment(s).
After the consultation, a written price quote and a copy of these appointment policies are given. Clients are never pressured to schedule a procedure. In fact, I urge clients to take the information home, think it over, discuss it with family and call me when they wish to proceed.
To reserve an appointment for a procedure, a $100 non refundable deposit is required. Payment methods accepted are major credit cards, check, cash and PayPal. The deposit will be applied toward the cost of the procedure. Pregnant or nursing women or anyone under the age of 18 will not be considered for tattooing. Please advise me now if any of these apply.
If an appointment must be rescheduled, a 24 hour notice is required in order to fill the opening in the schedule. The deposit will be applied toward the rescheduled appointment. However, if an appointment is a ‘no show’ or last minute cancellation, the deposit will not be refunded or reapplied.

The original session is called a Saturation visit. The client will check in for a visual assessment and if a following visit is required it is known as the Focus visit. If a focus is required it must be completed within 45 days of the Saturation visit and will cost no extra, however if a Focus visit is a no or scheduled past the 45 day limit, it will be charged at $100 per visit.
A Color Assessment appointment should be scheduled at the end of the first year and if a Touch-Up is needed, the charge is $200.00 per procedure. Depending on how many years between touch ups, the charge will be based on $200 per year. The cost will never exceed the original cost unless someone else has performed work on the area and correction is required.
All color fades and your cosmetic tattoo will require maintenance. The amount of fading depends on several variables including medications and topical treatments, but is primarily due to sun (ultra-violet) exposure; therefore measures should be taken to protect your permanent cosmetics from the sun.
You must consider this commitment to be permanent and understand that once applied, the process can never be fully reversed. “A tattoo can never be un-tattooed” There are risks in permanent cosmetic, as in any tattoo. The procedure is semi invasive in that the skin is broken and if care is not taken, infection can occur. You will receive an after care kit with instructions at the completion of the each visit. By following the instructions and using only the products in your kit, the possibility of infection are greatly reduced.
Your aftercare instructions will be read aloud to you as well as given to you in written form and your signature verifying that you understand the instructions will be required. You may not be under the influence of drugs or alcohol at the time of the procedure.
You will be required to sign a consent form as well as complete a medical profile. You must release by signature the use of your photos LiZa Sims, Always Perfect Permanent Cosmetics. If you wish your full face not to be used for advertising or educational purposes, please indicate so on the consent form. Eyeliner procedures must not wear mascara or other eye makeup during the week of healing. This is a MUST. Anyone who has received a bone or organ transplant must speak to their doctor about the procedure and follow his/her protocol.

Your permanent cosmetics will be vibrant for the first few days (especially lips). This is normal and will fade as the top layers of skin shed off. The shedding of the skin takes 3-5 days normally.
Remarks made by less than sensitive friends or family members may be met by asking them to reserve judgment until the process is complete.
I, the undersigned, received these appointment policies both verbally and in written form and I agree to comply with them. My signature is my consent to charge the aforementioned $100 non refundable deposit against my credit card should I decide to reserve an appointment by phone.
